Nurse Practitioner - Diabetes and Eating Disorders Program

Position Summary: 

Reporting to the Director of Clinical Services: Ambulatory Care, and the Program Manager for Outpatient Mental Health and Addictions, the NP within the both programs will provide clinical leadership and occupy a lead role in the comprehensive care of patients with diabetes and eating disorders across the lifespan. The NP will work collaboratively within two multidisciplinary teams. Working within their full scope of practice, the NP will be ordering and interpreting diagnostic tests, performing intakes, prescribing medications, education, monitoring, referring patients when required to other specialists and all other duties as assigned within the CNO Standards of Practice and policies of the TADH. 

Position Responsibilities:  

  • Provide comprehensive client care using advanced assessment skills and sound knowledge of clinical therapeutics for both diabetes and eating disorder diagnoses.
  • Direct, provide and evaluate as holistic client care based on advanced nursing and medical management knowledge and skills.
  • Perform and document comprehensive client assessments based on thorough history taking and physical examination.
  • Establishing and maintaining a therapeutic relationship which focuses on the needs of the patients.
  • Provide eating disorder specific therapy modalities to patients such as: CBT-T, CBT-E, and FBT.
  • Identify and analyze client’s physical and psychological health care problems.
  • Develop plan of care in collaboration with the team of professionals within both programs.
  • Identify health education needs of clients and families independently or in collaboration with the interdisciplinary team.
  • Identify, order and prioritize appropriate diagnostic test, medications and procedures.
  • Interpret laboratory/radiology and other test results.
  • Communicate with and interpret to clients/families and community health care members diagnosis, treatment plan, and related implications.
  • Accurately document assessment, plan, intervention, and evaluation of client/family responses in hospital chart.
  • Direct referrals to the appropriate health care professionals or services.
  • Provide individual counselling/instruction to facilitate client/family understanding of Diabetes and/or Eating Disorders.
  • Assist individuals and families in coping with Diabetes management and treatment plan by guiding and supporting while providing an environment that promotes independence.
  • Capture relevant and required data as defined by the Ontario Health, HQO, Accreditation or other quality improvement strategies.
  • Align improvement projects with the organization and monitor or adjust to achieve greater outcomes.
  • Perform other duties as required.

 

Position Requirements:

  • Completion of accredited post BScN, Nurse Practitioner program
  • Registered and in good standing with the College of Nurses of Ontario
  • Minimum of 2 years’ experience working with clients with acute and complex medical issues.
  • Working knowledge of developing rapport with clients who have poor distress tolerance and emotion regulation challenges.
  • Past experience working as an NP in an outpatient setting considered an asset
  • Experience in program development and education
  • Demonstrated knowledge of evidence based practices
  • Effective time management and ability to prioritize workload
  • Must possess the ability to work with minimal supervision and must follow standardized procedures.
  • Should be adaptable to cope with changing contacts with people, situations and conditions.
  • Must have employment history which demonstrates regular attendance at work.
  • Must possess the ability to provide effective services to clients/patients in both English and French. Individuals who do not meet the language requirements of this posting are invited to submit their résumé for consideration.

  • Starting salary of $60.99/hour; $118,930.50/year (As per Step 1 of the ONA collective agreement.)
  • Option to enroll into Healthcare of Ontario Pension Plan.
