Reporting to the Executive Director, East End Family Health Team, the Registered Social Service Worker will help individuals, couples, families, groups, and the community develop the skills and access the resources they need to enhance their well-being and social functioning.

What you will be doing:

  • Facilitate pathways and referrals to appropriate community-based services and supports;
  • Advocate to community and social service agencies on behalf of patients;
  • Coordinate care, which involves scheduling appointments, arranging transportation, and organizing care;
  • Provide support by acting as a liaison between the patient and their medical team to ensure, among other things, that cultural needs are being met and that the patient understands their treatment plans;
  • Provide education and support for patients (and family members) as they learn about their diagnosis, the importance of follow-up care, and the implications the diagnosis can have on their lifestyle and wellbeing; and
  • Help to develop and implement new support programs at the EEFHT in collaboration with team members and community partners.

What you will need to be considered:

  • Two (2) year diploma in Social Service Worker, Social Work, Addictions and Mental Health, or a related/relevant field;
  • Minimum one (1) year experience working in a position that involves directly interacting with the public;
  • Current registration in good standing with the College of Social Workers and Social Service Workers; and
  • Ability to provide a clear criminal record check for the vulnerable sector.

The successful candidate will be a critical thinker, compassionate and well organized. They will also have strong communication (verbal and written) skills, conflict resolution skills, and time management skills.
